Origin’s website, located at www.theoriginway.com, mobile applications, and other products and services (collectively, the “Services”), are owned and operated by The Origin Way, Inc. and its affiliates (individually or collectively referred to herein as "Origin", “we,” or “us”) for your needs. Please read these terms of use (“Terms of Use”) carefully because they set forth the important terms you will need to know about the Service. Your use of the Services constitutes your agreement to all such terms, conditions, and notices. In the event that you do not agree with these Terms of Use, do not access or use our Services.
In these Terms of Use, the terms “you” and “yours” refer to the person using the Services. Use of the Services is governed by these Terms of Use, and by using the Services you agree to abide by these Terms of Use as well as all applicable laws and regulations. If you do not accept and agree to be bound by any of these Terms of Use, you are not authorized to access or otherwise use the Services or any information or Content contained in the Services, as defined in these Terms of Use. Your access to and use of the Services constitutes your acceptance of and agreement to abide by each of these terms and conditions set forth below.

Service Description
We are pleased you have chosen Origin Physical Therapy (FL), PLLC, Origin Physical Therapy (CA), Inc., Clampett Physical Therapy (NY), PLLC, Sullivan Physical Therapy, LLC, Origin Physical Therapy (NJ), LLC, Fiat Physical Therapy, LLC, To The Core Physical Therapy & Conditioning LLC, and other professional practices, companies and individuals (“Origin Professionals”) to provide you with physical therapy (“Clinical Services”). Origin Professionals engage independent doctors of physical therapy and other professionals (“Physical Therapists”) to provide Clinical Services to you. Origin does not practice medicine or provide physical therapy, and is independent from Origin Professionals and the professionals who provide Clinical Services. Throughout these Terms of Use, the terms Clinical Services and Services may be used interchangeably.
All Physical Therapists providing Clinical Services hold all required professional licenses issued by the professional licensing board in the states where they practice. You may report a complaint to the Clinical Services by contacting the professional licensing board in your state of residence. For your convenience, the contact information for the state professional licensing licensing boards governing physical therapy can be found on The Federation of State Boards Physical Therapy website.
The Services contains certain pages, sections, services, and content that are only available to registered users of the Clinical Services, including a means to obtain video-chat or electronic messaging consultations for a range of health issues by facilitating the connection between you and Physical Therapists through use of the Services.
Eligibility
To utilize the Services, you must 1) create and register for an account; 2) be at least 18 years of age; 3) be a resident and agree to access the Services from the United States; 4) accept these Terms of Use; 5) and meet certain physical and medical criteria. You may not create multiple accounts or create accounts from outside the United States. If you are below the age of 18, you may be eligible for the Clinical Services as determined by the Physical Therapist.
Chaperones
Origin supports you to request a chaperone (“Chaperone”) to be present during interactions with your Physical Therapist. A Chaperone is a trained person who has been oriented to support and be a witness for a patient and a provider during a sensitive exam or procedure and who does not report directly to the care provider who is performing the examination. At this time, Origin is unable to provide Chaperones. If you bring a Chaperone to your appointment, you agree to be financially and legally responsible for your Chaperone.
Payments
By accessing the Clinical Services, you agree to our Financial Policy at https://www.theoriginway.com/financial-policies.
By using the Clinical Services, you authorize Origin to automatically process your credit card as the “Card on File” for any amounts owed. You understand this authorization will remain in effect until (i) the expiration of the credit card account, or (ii) you revoke your authorization of this form by submitting a written request to Origin. You understand the Card on File may be used to cover any expenses incurred related to the Clinical Services including without limitation: copays, coinsurance, deductibles, outstanding treatment charges, missed or late cancellation fees, equipment, and supplies. You agree to have a Card on File before you access or utilize the Clinical Services. Cancellation Fees and patient balances will be automatically charged. These terms serve as notice of our Cancellation Fees.
To the extent that you purchase specific services or products via the Services, including without limitation payments to Practitioners, then processing services for such payments may be provided by Stripe and are subject to the Stripe Connected Account Agreement, which includes the Stripe Terms of Service (collectively, the "Stripe Services Agreement"). By agreeing to these terms you agree to be bound by the Stripe Services Agreement, as the same may be modified by Stripe from time to time. As a condition of the Services enabling payment processing services through Stripe, you agree to provide the Services accurate and complete information about you and your business, and you authorize the Services to share it and transaction information related to your use of the payment processing services provided by Stripe.

Accounts and Registration - Security

You are solely responsible for maintaining the confidentiality of your account and password, if any, and you accept responsibility for all activities that occur under your account. You are prohibited from violating, or attempting to violate, the security of the Services including, without limitation, (a) accessing data not intended for such user or logging onto a server or an account which the user is not authorized to access; or (b) attempting to probe, scan or test the vulnerability of a system or network or to breach security or authentication measures without proper authorization; or (c) accessing or using the Service or any portion thereof without authorization, in violation of these Terms of Use or in violation of applicable law. Any such violations may result in criminal and/or civil penalties against you. Origin may investigate any alleged or suspected violations and if a criminal violation is suspected, Origin will cooperate with law enforcement agencies in their investigations.
